J.O.
If you get a chance, read Tony Kern's book "A Darker Shade of Blue". Kern, a retired USAF pilot, was friends with Bud Holland in his early days in the air force and was shocked to be hearing some of the stories of Holland's command style and recklessness. When he got to the truth, he found a rogue whose acts were well known and that repeated attempts to have him grounded, including by the man in the right seat on that fateful day, were brushed aside because of Holland's status and popularity among the senior command. Kern's book is an excellent look into the command structure.
