Main wheels do not fall off unless there is a reason. ncident: Canada A319 at New York and Toronto on Feb 18th 2020, dropped main wheel on departure By Simon Hradecky, created Tuesday, Feb 18th 2020 22:27Z, last updated Tuesday, Feb 18th 2020 22:27Z An Air Canada Airbus A319-100, registration C-GAQL performing flight AC-715 from New York La Guardia,NY (USA) to Toronto,ON (Canada) with 120 passengers and 5 crew, departed La Guardia's runway 13, climbed to FL300 enroute to Toronto. On approach to Toronto the crew reported a possible problem with the landing gear and performed a low approach to Toronto's runway 24L to have the landing gear inspected from the ground which revealed the right outboard main wheel was missing. The aircraft subsequently positioned for a landing on runway 23 and landed without further incident and stopped on the runway. boestar
historically it has been related to improper torquing of the Wheel Nut leading to a bearing failure or not properly safetying the lock bolts on the nut correctly. There have even been cases of the wheel nut spacers being left out on re-installation of a wheel. There have been many cases of this at all airlines at some point. The 767-200 was prone to failure and it was narowed down to a torque problem. A re-torque procedure was adopted that the problem vanished. Investigation will lead to the root cause eventually and mitigating action will be introduced. Thats how aviation works. You don't know its wrong until it fails. I've also been around this industry long enough to know that sometimes, things just break, even when everything else was done right.

J.O.
Failed bearing suspected after A319 wheel loss. Canadian investigators have observed an apparent failed outer bearing on the main landing-gear assembly of the Air Canada Airbus A319 which lost a wheel before arrival at Toronto. Transportation Safety Board of Canada says images of the aircraft, which arrived from New York LaGuardia on 18 February, indicate that the failed bearing allowed the wheel assembly to "slip out of the axle". https://www.flightglobal.com/safety/failed-bearing-suspected-after-a319-wheel-loss/136928.article
