DEFCON
" #BUSINESS NEWS SEPTEMBER 21, 2017 / 9:54 AM / 21 DAYS AGO UAE investigating Emirates A380 'serious incident' in Moscow Alexander Cornwell 3 MIN READ (Reuters) - The United Arab Emirates’ (UAE) civil aviation regulator is investigating why an Emirates [EMIRA.UL] Airbus (AIR.PA) A380 superjumbo flew dangerously low when approaching Moscow’s Domodedovo Airport last week. The General Civil Aviation Authority (GCAA) said its air accident investigation unit is treating the Sept. 10 episode as a “serious incident”, an internationally recognized category which means an accident “nearly occurred”. https://www.google.ca/amp/mobile.reuters.com/article/amp/idUSKCN1BW1WA
Trader
One of many recent incidents most have which been 'hidden' internally (or, to be fair, not picked up by media because ATC didn't report it). The results of years of 95-100 hour months and fatigue and a dramatic decline in the experience levels of new hires. The crash last year (which I think many of us saw coming) hasn't changed a thing. In fact the companies response to the DME incident has been to 'blame and attack' the pilots as it was after the crash. Pilots have now been warned that failures or a poor sim performance will result in serious consequences. No responsibility from the side of management with regards to schedules. lack of training etc. Gotta love modern management!

Sure would like to see some facts and figures: Duty time of crew, experience of crew, time of day, was the aircraft on auto and perhaps a software failure not noticed by the crew , or ?????????
